    If your ferritin isn’t improving despite supplemen If your ferritin isn’t improving despite supplementation, the issue may not be intake.

It may be absorption.

Iron deficiency has three primary drivers:

1️⃣ Blood loss
2️⃣ Inflammatory sequestration
3️⃣ Malabsorption

In clinical practice, I often see women repeatedly prescribed higher doses of iron without investigating the underlying mechanism.

Iron is absorbed primarily in the proximal small intestine. If there is: 

• Chronic bloating
• Reflux
• IBS
• Untreated Helicobacter pylori infection
• Small intestinal bacterial overgrowth
• Coeliac disease
• Low stomach acid 

Absorption may be compromised. 
This impairs iron uptake by affecting stomach acid, mucosal integrity, or inflammatory signalling.

In other cases, ferritin may appear normal, yet serum iron and transferrin saturation are low. This suggests inflammatory blockade — where the hormone hepcidin reduces iron absorption and traps iron in storage.

More supplementation does not override hepcidin.

In fertility and pregnancy care, this matters.

Iron supports:

• Ovulation
• Endometrial oxygenation
• Placental development
• Postpartum recovery

If ferritin won’t rise, the question is not “How much more iron?”

The question is “Why isn’t it being absorbed?”

Iron deficiency is rarely random.

It’s a signal.
    If you’ve ever been told “Your iron is low” witho If you’ve ever been told
“Your iron is low”

without context…

You’re missing the real story.

Iron status is not one number.
It’s a pattern.

Here’s what actually matters:

Ferritin → your storage tank.
Low = depleted.
But it rises with inflammation.

Serum iron → what’s circulating today.
Fluctuates. Not diagnostic alone.

Transferrin → the transporter.
High transferrin + low ferritin = classic depletion.

Transferrin saturation → availability.
<20% often explains fatigue, hair shedding and poor concentration.

Now the crucial part:

CRP.

Inflammation raises hepcidin — which blocks iron absorption and traps iron in storage.

So you can have:
Normal ferritin
Low serum iron
Low saturation
High CRP

That’s functional iron deficiency.

Different mechanism.
Different treatment.

And remember — by the time haemoglobin drops, deficiency has usually been present for a while.

Iron is regulated.
Not random.

Decode the pattern — not just the number.

They regulate hormones that influence:
- Brain chemistry
- Bone density
- Cardiovascular function
- Metabolism
- Immune health

Whether or not fertility is part of your plan, ovarian function plays a central role in long term health outcomes.

If you’re trying to conceive, understanding when you ovulate isn’t helpful — it’s essential.

Once an egg is released at ovulation, it survives for just ⭐️12–24 hrs⭐️ That’s it. One brief window each cycle where fertilisation can occur.

Sperm, on the other hand, are far more patient. In healthy cervical mucus, they can survive for up to 5 days, waiting in the cervical crypts for the egg to appear.
👉 This is why timing matters.

Research consistently shows that the highest chance of conception comes from having sex the day before ovulation, not after. 
From large prospective studies (Wilcox et al., NEJM 1995):
• 5 days before ovulation: ~10% chance
• 3 days before: ~15–20% chance
• 2 days before: ~25–27% chance
• 1 day before: ~30–33% chance (highest probability). Some studies have quoted up to 41% chance
• Day of ovulation: ~12–15% chance
• Day after ovulation: ~0–5% chance (egg usually no longer viable)

👉By the time ovulation has already happened, the window may already be closing.
And the egg is re-absorbed. Just like that.
So if you’re only trying once you think you’ve ovulated, you could be missing your most fertile day entirely.
Learn your body.
Track your signs.
Understand your fertile window.
Because conception isn’t just about ovulating — it’s about timing intercourse to meet the egg when it actually arrives.
